  • 12.A. As appropriate for the laboratory services performed, a current CLIA Certificate of Waiver, and/or a current Certificate for Provider Performed Microscopy Procedures (PPMP), and/or a current Certificate of Registration, Compliance or Accreditation is present.
  • 12.B. If the organization has obtained a CLIA Certificate for Provider Performed Microscopy Procedures, or a CLIA Certificate of Registration, Compliance or Accreditation, services are provided under the direction of a pathologist, other physician, or other qualified individual as delineated under CLIA.
  • 12.G. Pathology and medical laboratory services provided adequately support the organization’s clinical capabilities.
